Deferral or Reapplicant Readiness and Reflection Form

This survey contains sixteen (16) open-ended questions and should take an hour to complete. We recommend that this is completed on a desktop or laptop, opposed to a mobile device (phone, iPad, tablet). 

In addition to completing the form responses, you will be prompted to complete the following: 

  • Disclosing any student conduct infractions
  • Upload the following three (3) documents to the documents section of your Handshake profile:
    • Updated resume/CV 
    • Personal statement (5,300 character limit)
    • Copies of unofficial transcripts from all undergraduate institutions you have attended
  • Letters of Recommendation (LOR’s): 
    • Request four (4) Letters of Recommendation from individuals who can write a strong letter of support on your behalf. 
    • We strongly recommend that you have at least two (2) URI faculty letters, and that one of them is an instructor from the science(s) (Biology, Chemistry, Physics, Neuroscience are examples).
    • Please direct your letter writers to submit them on our website.
