JWU Physician Assistant Applicant Readiness and Reflection Form

This survey contains a series of open-ended questions and should take an hour to complete. We recommend that this is completed on a desktop or laptop, opposed to a mobile device (phone, iPad, tablet). 

In addition to completing the form responses, you will be prompted to complete the following: 

  • Disclosing any student conduct infractions
  • Upload the following three (3) documents to the documents section of your Handshake profile:
    • Updated resume/CV 
    • Personal statement (5,300 character limit)
    • Copies of unofficial transcripts from all undergraduate institutions you have attended
